We believe every person deserves independence and the ability to stay in their own home. If you or your loved ones have suffered a major medical event or are experiencing problems keeping up with daily activities, home health can help.
Our home health team works with each patient to develop a customized treatment plan based on their current health and goals.
Home health is usually a temporary treatment for an acute medical need, such as after surgery. Our main focus is to keep people out of the hospital. With our skilled nurses and therapists, we can help our patients stay happy and healthy at home. Haven is owned and operated by clinicians, and we make it a priority to take excellent care of our staff, patients, and clients.
There is a lot of misunderstanding around hospice care. Our goal at Haven is to give people the best possible care, and part of that is making sure patients know what their choices are. Most people come to hospice services too late. While the conversation can be difficult to have, hospice care is simply about changing the focus of care from length of life to quality of life.
Haven provides comprehensive support and education to families and caregivers, equipping them with the necessary skills and knowledge to care for their loved ones effectively at home. This training covers a range of topics, including medication management, safety protocols, daily care routines, and techniques for handling emergencies. It aims to enhance caregivers’ confidence and competence, ensuring they can provide high-quality care while also addressing their emotional well-being. Through personalized guidance, practical demonstrations, and ongoing support, the program fosters a collaborative approach to home health care, promoting better outcomes for both patients and caregivers.
