Job Summary:

The LPN provides skilled nursing services to patients in their homes to promote the patient’s plan of care including to assess, plan, implement, document, and evaluate care to patients and families. The LPN supplements the nursing care needs of that patient as provided by the RN. These services are performed in accordance with the physicians’ orders and in collaboration with Haven Home Health RNs, therapists, and other clinicians.

Duties and Responsibilities:

  • Recognize and report changes in the patient’s condition to the RN supervisor.
  • Complete timely and accurate clinical notes including addressing patient’s progress.
  • Assist the RN in carrying out the patient plan of care.

Qualifications, Knowledge, Skills, and Abilities:

  • Current Missouri LPN license.
  • At least one (1) year recent LPN experience in acute, long term or home care preferred.
  • Home healthcare experience preferred.
