Job Description:
The LPN participates in the day-to-day coordination and delivery of skilled nursing services and performs related divisional teaching/visit duties to ensure the highest quality of care.

 Duties and Responsibilities:

  • Develops and maintains on-going relationships with management, department staff, case workers, patients, and family members in order to best meet the needs of clients.
  • Ensures that delivery of nursing services is compliant with company/state policies.
  • Available resource person to other health team members in the identification and fulfillment of patient needs.
  • Maintains liaison with the community’s health-related resources.

 Qualifications, Knowledge, Skills, and Abilities:

  • Graduate of approved School of Professional Nursing – or –
  • Current state Licensed Practical or Registered Nurse license.
  • Pediatric nursing experience preferred.
