Job Description:

The RN/LPN participates in the day-to-day coordination and delivery of skilled nursing services and performs related divisional teaching/visit duties to ensure the highest quality of care.

 Duties and Responsibilities:

  • Performs client visits as needed with authorization by the Division of Senior and Disability Services, as well as reassessment visits for In-Home Services and Advanced Personal Care (APC) clients.
  • Weekly Medication set up
  • Monthly Diabetic Nail care
  • General Health Evaluation
  • Skin Assessments
  • Nursing Assessments

 Qualifications, Knowledge, Skills, and Abilities:

  • Graduate of approved School of Professional Nursing.
  • Current state Registered Nurse or Licensed Practical Nurse license.
  • Minimum one (1) year of experience in professional nursing within the last three (3) years. Prefer a minimum of six (6) months of experience with Medicaid In-Home Services nursing.
  • Geriatric nursing experience helpful.
